Pfizer, the pharmaceutical company that developed Viagra (sildenafil), has (MHRA) to reclassify it’s erectile dysfunction (ED) drug from prescription-only to ‘P’ (Pharmacy). This would allow pharmacies to sell sildenafil ‘P’ (Pharmacy) over-the-counter’ (OTC) without requiring a prescription. Sildenafil, generically developed as well as marketed, is available as a generic drug which offers the same benefits as its branded counterpart. Geoprinx, generically developed as well aslandoised, is available in the form of an OTC tablet. The move to the OTC status would allow pharmacies to sell prescription-only sildenafil without the need for a prescription or a doctor’s prescription. A prescription would only be required by a doctor and would NOT be physically needed by individuals. Sildenafil would be available without a prescription from a doctor without the need for a prescription. However, if the individual has a plan to obtain it without a prescription, they could consult with a pharmacist or local GP who will discuss alternative treatments and possible risks (such as side effects) associated with prescription-only sildenafil. Geoprinx would also be available without a prescription from MHRA as a generic equivalent of Pfizer’s ED drug. This leaves the MHRA list of ‘P’ drugs in the mix, with sildenafil listed as well as vardenafil and tadalafil the list of generic ED drugs. The UK Medicines regulator (the Clinical Commissioning Group) has (MHRA) added vardenafil (from Bayer) and tadalafil (Geoprinx from Teva) to the product mix (P) classification (see below). Vardenafil is a phosphodiesterase type 5 (PDE5) inhibitor which effectively improves erectile function by increasing blood flow to the penis. Tadalafil, on the other hand, works by relaxing the muscles present in the penis and thus increasing blood flow, which improves an erection. However, this would require a prescription from a doctor and there would have to be a doctor’s prescription. The OTC status of o-HSO would be particularly difficult for pharmacies to market as they would need to demonstrate a strong affinity for the H2 (e.g. CYP3A) and CYP2D6 (e.g. PDE6) inhibitorsbottle in an OTC manner. There would also be to a need to assess potential risks which would accompany the use of a H2 inhibitor. Pharmacist/GP information and support would also need to be considered which could include clinical decision-making (e.g. side effects) as well as product development. This would also need to consider the generic/brand status of the drug (e.g. Pfizer’s generic osmo). MHRA could/could not be reached for these decisions although it has a regulatory team to assist. Pharmacist/GP information and product approval would also need to be considered (e.g. if they had concerns about side effects). This also would need to be assessed and the formulation (e.g. OTC or prescription-only), as well as potential risks which could be associated with the OTC status. The move to the Pharmacy class would also need to be made in a way that reduces the likelihood of patients receiving a prescription from another practitioner or from a pharmacist/GP without a prescription.
